New Patient Registration
If you live within our practice area you can register by coming into the surgery and our reception staff will be happy to guide you through the procedure.You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with our healthcare assistant to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please note: From 1st November 2018 we can not accept any new registrations from patients living outside of our practice area. Any currently registered patients who move out of area after this date will have to register at a practice local to their new address.
Change of Personal Details
If you change your name, address or telephone number, please pick up a form at reception and return it as soon as possible, so our admin staff can update your details immediately.
